1.  Municipal Treasurer's Office (for assement)

•  Police Clearance
•  Business form
•  Previous Mayor's Permit (for renewal)
•  Financial Statment (for renewal of business)
•  Print out copy of Tax Computation (for tax due)

2.  Municipal Health Office

•  Sanitary Permit / Health Certificate

3.  Clearance from Regulating Offices (if applicable)

•  Bureau of Internal Revenue
•  Office of the Building Official
•  SSS
•  Bureau of Fire Protection

Service:  Request for Solemnization of Marriage

Who avail of the service - All bonafide residents of Ginatilan

Schedule of Service Availability - Monday to Saturday, 8:00 AM - 5:00 PM

Requirements - Original copy of Marriage License issued by the Municipal Registrar and Official Receipt issued by the Municipal Treasurer's Office

Fees - P 200.00 (both Filipino Citizen - male and female)

           P 500.00 (for  Filipino Citizen married to a Foreigner)

Service:  Application for Water Installation Permit

Who may avail of the service - All bonafide residents in Ginatilan

Requirements - Official Receipt issued by the Municipal Treasurer

Fees - P 250.00

Service:  Application for Tricycle Operator's Permit

Who may avail of the service - All bona fide residents in Ginatilan

Requirements - Official receipts issued by the Municipal Treasurer and Certificate of Registration

Fees - P 120.00

Service:  Special Permit - Benefit Dance

Who may avail of the service - All bona fide residents in Ginatilan

Requirements - O.R. issued by the Municipal Treasurer and Certification of Barangay Captain

Fees - P 50.00 for upper barangays, P 100.00 for lower barangays
